– Transcript of Records

– Certificate of Good Moral Character

Eligibility Documents

– Report of Rating

– PRC Eligibility (with authenticated PRC ID – must be authenticated in CY 2025)

Clearances (all required)

– Barangay Clearance

– Police Clearance

– Mayor’s Clearance

– Prosecutor/Fiscal Clearance

– NBI Clearance

– MTC, METC, and RTC Clearances
